The deceased, a 23-year-old patient at the dental clinic of a major hospital, was getting her lower left wisdom tooth removed. After the extraction, she continued to be in pain and returned to the clinic several times throughout the next two weeks. Due to swelling in her throat, surgery was suggested. Following the surgery, a tube was placed in her nose to her trachea to help breathing.
She was moved to the intensive care unit to be monitored. Her infection began to improve within a few days, but the breathing tube was still her “life line.” One morning, a nurse was bathing the patient when the tube became dislodged and moved, cutting off her oxygen for approximately 10 minutes and causing fatal brain damage.
