This month the National Traffic Safety Board (NTSB) issued its finding and probable cause of an Arizona helicopter crash in Cave Creek on February 14, 2010, reports the Sonora News.
Killed in the crash were a 64-year-old man, his 40-year-old wife, her 38-year-old brother, the couple’s 5-year-old daughter, the 63-year-old pilot, and two dogs.
The helicopter was en route from the couple's ranch in Parks, Arizona, to the Scottsdale Airport. A ranch foreman saw the 64-year-old man board the helicopter and occupy the left cockpit seat. He then saw the 5-year-old daughter sit on her father’s lap.
Both the left and right cockpit seats were equipped with dual flight controls.
When the helicopter crashed, witnesses say that it landed “slightly nose-down and slightly left-bank attitude” before it was ultimately consumed by a post-crash fire.
The NTSB study of the accident determined that it was highly likely that the man’s daughter, inadvertently stepped on a control with her left foot and displaced it to the full down position.
The NTSB stated that the pilot failed to use proper cockpit discipline when he allowed the 64-year-old man to operate the helicopter’s controls and when he allowed the 5-year-old girl to sit on her father‘s lap.
